About Us... Community Care Management began in 1993 as a not-for-profit organization formed to manage a pioneering integrated system of care for troubled children and their families. The system's goal was to fuse the resources of individual families, social service agencies, professional service providers and a variety of community-based organizations into solutions that truly worked. As a result, children were not sent away to institutions with little chance of learning the skills they needed to live with their families and in their communities. Public agencies and private providers were asked to rethink their standards of counseling and care. Community and faith-based organizations were invited and encouraged to participate. Most important of all, families were involved as the key decision-makers in the care of their children. Success with that initial program and the need to incorporate system design and start-up consultation, training, administrative services, evaluation and information systems development into the process, led to formation of Community Care Systems. Communities wishing to create managed systems of mental health, human services and social support for high-need children and families had a model and, importantly, the technical assistance they needed to get started. Community Care's philosophy of amassing the best empirical information and most critical resources into a superior model of care extends to the composition of our own team of experienced professionals. Community Care's senior staff includes people with backgrounds in health care and human services consulting, social work and social services administration, mental health care and counseling, non-profit agency management, clinical training, health services research, software development and data management. Their ability to identify and help communities meet the needs of at-risk and vulnerable populations is built on time spent in the field, doing the work and finding better ways to reach better outcomes. |
